Will all portals in a certain group appear in the same city?

Not necessarily. Not all cities are the same, not all geographic regions are the same, and therefore each Special Field Operation will be unique. There is no guarantee that these Portals will be inside the physical or political boundaries of a single city or district etc.

Local Time refers to the timezone of the group of Portals associated with that Special Field Op. It is safe to assume that all three individual Portals that comprise a group for a Special Field Op would exist in the same timezone.

Times for San José, Costa Rica are in CST UTC-6, which is Local Time for that city. The Anomaly Intel document previously stated that CST was UTC+6. This error has been corrected and the Anomaly Intel doc for San José has been updated.
